docs: make history arrowhead explicit - #13

Summary

• Replace the curved history connector marker with an explicit filled triangle at the endpoint.
• Add a cache-busting revision to the README diagram URL.
• Leave the straight connector arrowheads unchanged.

Validation

• Rendered the SVG at README scale with rsvg-convert.
• git diff --check passed.
